APPLICATION FORM CHECKLIST

• REGISTRATION FORM IS COMPLETED AND SIGNED
• PROOF OF PAYMENT
• PROOF OF IDENTITY (ID COPY)
• PROOF OF IDENTITY (PARENT/ GUARDIAN, students under 18 years old)

ALL OUR TUTORIALS/CLASSES MAY INCLUDE STARTER KIT

FINANCES

• Registration fees cover enrolment & administration costs only,
• Tutorial Fees includes the following:
• Student must pay a deposit of 50% or more in order to secure their Tutorial Slot
  • Full payment is required in order to start attending Tutorial classes,
  • Students who wish to attend 20 days or more, monthly installments are available,
• Registration Fees and Deposit are non-refundable,
  • Students who wish to cancel before tutorials start, will be charged 40% of the total fees,

ACCOMODATION AVAILABLE

• R250 per night includes transport, breakfast and supper,
• Free accomodation for sharing only,
• Transport will only be used to and from the accommodation for tutorials/ classes purposes only.

TERMS AND CONDITIONS

UPON SIGNING THIS REGISTRATION FORM THE STUDENT AGREES TO THE FOLLOWING:

• Attend Tutorials held Monday-Friday, And weekends upon arrangement.
• Each day you will attend tutorial for a maximum 8 hours,
• Wear professional attire facemask, dark clothing, black t-shirt, denim jeans or skirts,
• Student needs to arrange their own food and transport prior to the tutorials commencing,
• To respect the rules and codes of conduct at MYFACE Beauty & Stylist Academy.
